Breast Augmentation Techniques

Periareolar Breast Augmentation (Incision at the Areola Edge)

In this preferred method, the incision is made along the natural edge of the areola. The scar is almost invisible, blending with the areola’s natural color. Although the incision length is limited by the size of the areola, it provides an aesthetically discreet access for implant placement.

Inframammary Breast Augmentation (Incision Under the Breast)

The inframammary incision beneath the breast fold is suitable for almost any implant size. It allows for precise and optimal placement of the implants. In rare cases, very heavy implants may slightly shift the breast fold over time.

Transaxillary Breast Augmentation (Incision in the Armpit)

This technique leaves no scar on the breast itself. The incision is discreetly placed in the armpit, making it ideal for patients who prefer a scar-free look on the breast. It is also suitable for larger implants, though placement via the armpit requires greater surgical experience due to the more complex access.

Autologous Fat Transfer (Fat Grafting / Lipofilling)

As a gentle alternative to silicone implants, breast augmentation with autologous fat is an option. This technique is ideal for those seeking an increase of approximately one cup size. Excess fat from the hips, thighs, or abdomen is harvested and used for the augmentation, providing a dual benefit of contouring donor areas while enhancing breast volume naturally.

Breast Implant Placement

Breast implants can be positioned either subglandular (above the chest muscle) or submuscular (below the chest muscle), depending on individual anatomy and desired results:

  • Subglandular (above the muscle):

    • Direct contact with breast tissue

    • Creates a soft, natural contour

    • Shorter recovery time

  • Submuscular (below the muscle):

    • Less visible implant edges in patients with thin breast tissue

    • Reduced risk of visible rippling or contours

    • Often recommended for very slender patients

The choice of implant placement is personalized to ensure a natural, harmonious, and long-lasting result.

BREAST IMPLANTS

Proper preparation is essential for a safe procedure and a smooth recovery. Key steps include:

  • Medical evaluation by a general practitioner or internist to assess overall health

  • Discontinuation of blood-thinning medications (e.g., Aspirin®, Thomapyrin®, Thrombo ASS®) at least one week before and after surgery to reduce the risk of bleeding

  • Limiting or avoiding alcohol and nicotine before and after the procedure, as these substances can impair wound healing

  • Consultation on implant size, shape, and surgical technique to ensure optimal aesthetic results

  • Optional mammography or breast ultrasound to rule out other breast conditions prior to surgery

  • Surgery duration: Approximately 1 – 2 hours, depending on the implant type and placement

Proper postoperative care is essential for optimal healing and long-lasting results:

  • Wear a supportive bra for 4 – 6 weeks to stabilize the implants

  • Avoid heavy physical labor and lifting objects, especially above shoulder level, for 4 – 6 weeks

  • Refrain from sports or intense physical activity for approximately 6 weeks

  • Avoid sauna or steam bath visits during the early healing phase

  • Mild swelling and tightness in the breasts are normal and part of the healing process

  • Attend regular follow-up appointments to monitor recovery

  • Sutures do not usually require removal, as self-dissolving stitches are typically used

  • Sun protection: Use sunscreen with SPF 50 or higher on the incision areas to minimize scar visibility

  • Return to work: Most patients are able to resume work after a few days, depending on the type of activity

Breast augmentation is generally considered a safe and routine procedure. Temporary effects after surgery may include:

  • Swelling and bruising

  • Mild pain or tightness in the breasts

  • Temporary changes in nipple or breast sensation

  • Scarring, which usually heals inconspicuously

Serious complications such as infection, implant displacement, or postoperative bleeding are rare.

Over time, the shape, position, or condition of the implants can change:

Capsular Contracture
  • The body can form a fibrous capsule around the implant, which may harden

  • Possible effects include changes in breast shape, tightness, or mild discomfort

Implant Displacement or Rotation
  • Particularly with larger implants, the position may shift

  • This can lead to asymmetry in breast shape

Rupture or Material Fatigue
  • Silicone implants may become damaged over time

  • Modern implants are highly durable and long-lasting

Changes Due to Aging, Pregnancy, or Weight Fluctuations
  • Breast shape and volume may change independently of the implant

Rare Complications
  • Infections, bleeding, or reactions to the implant material are uncommon but possible

How long do the results last?

Breast augmentation is generally considered permanent, although implants may rarely need to be replaced over time.

Is the procedure painful?

Postoperative discomfort is usually moderate and can be effectively managed with pain medication.

When will I see the final result?

Once swelling has subsided, typically 3 – 6 months after surgery, the final breast shape becomes clearly visible.

Are the scars visible?

Incisions are made in discreet locations (inframammary fold, around the areola, or in the armpit) and fade significantly over time.

Can I breastfeed after breast augmentation?

With proper implant placement, breastfeeding is usually possible.

How often should breast implants be checked?
