Surgical eyelid correction (blepharoplasty) is a well-established procedure in aesthetic surgery and can be performed for various reasons. Types of Eyelid Surgery (Blepharoplasty)

In eyelid surgery (blepharoplasty), a distinction is made between upper eyelid surgery and lower eyelid surgery. The choice of technique depends on the individual anatomical characteristics as well as the desired treatment outcome.

Lower eyelid surgery is primarily performed to treat under-eye bags, wrinkles, and skin laxity in the lower eyelid area. The procedure typically takes approximately 1 to 1.5 hours. Depending on the clinical findings and the patient’s preferences, surgery may be carried out under local anesthesia, conscious sedation, or general anesthesia upon request.

Upper eyelid surgery is most commonly used to correct drooping eyelids, excess skin, and functional limitations of the visual field. It is usually performed on an outpatient basis under local anesthesia and takes about one hour. The procedure is generally gentle and low in discomfort, with most patients experiencing minimal sensation during surgery.

  • To ensure optimal preparation and recovery, blood-thinning medications such as Aspirin®, Thomapyrin®, or Thrombo ASS® should be discontinued one week before the procedure and for one week afterward — always in consultation with your physician.

  • In addition, patients are advised to limit nicotine and alcohol consumption in the period before surgery, as this supports proper wound healing and helps achieve the best possible results.

  • During upper eyelid surgery, excess skin and fatty tissue are gently removed, and the weakened eyelid muscle is selectively tightened. The procedure is performed pain-free under local anesthesia. The incision is discreetly placed within the natural crease of the upper eyelid, ensuring that scars are typically barely visible.

    The result: After upper eyelid surgery, the eyes appear more open, brighter, and visibly younger, giving the overall appearance a fresher and more alert look.


  • During lower eyelid surgery, fat deposits at the transition between the lower eyelid and the cheek area are either removed or repositioned. In addition, excess skin is removed and the weakened eyelid muscle is tightened to effectively reduce under-eye bags and fine wrinkles.

    The procedure is largely pain-free and the incision is placed either just below the eyelashes or inside the eyelid, resulting in minimal to no visible scarring. Lower eyelid surgery can create a noticeably smoother contour and give the eye area a more rested, youthful appearance.

  • The first plaster change (steri-strips) usually takes place after approximately 3 days.

  • Sutures are removed after 7 – 10 days, depending on the individual healing process.

  • To promote rapid and complication-free healing, we recommend the following:

    • Cooling the eye area, ideally with a specialized cooling eye mask, to reduce swelling

    • Lymphatic drainage treatments to support healing and help swelling subside more quickly

    • Avoiding sports and strenuous physical activity during the first 14 days after eyelid surgery

    • During the healing phase, we strongly advise avoiding smoking and alcohol consumption to minimize the risk of complications.

    These measures significantly contribute to faster recovery and an optimal aesthetic result.

  • After eyelid surgery, temporary mild to moderate swelling and bruising may occur, particularly in the lower eyelid and cheek area. These effects are normal and usually subside within one to two weeks.

  • In some cases, patients may experience a feeling of tightness, temporary numbness, or mild eye dryness. These symptoms are generally temporary and resolve on their own.

  • In rare cases, particularly after lower eyelid surgery, temporary disturbances of tear production or complete eyelid closure may occur. These changes usually resolve quickly.

  • Very rarely, wound healing complications, infections, or noticeable scarring may develop.

    Careful postoperative care and adherence to medical instructions significantly help to minimize potential risks.

Does health insurance cover the costs of a blepharoplasty?

A partial cost coverage by health insurance may be possible under certain conditions. This usually requires a visual field test performed by an ophthalmologist, demonstrating that the visual field is functionally impaired due to drooping eyelids.

Is eyelid surgery painful?

Eyelid surgery is generally associated with minimal discomfort. Thanks to local anesthesia, most patients experience little to no pain during the procedure.

How long is the downtime after eyelid surgery?

After eyelid surgery, the recovery period is typically around 10 to 14 days. During this time, mild swelling or bruising may still be visible.

After approximately 14 days, most patients are fully socially and professionally active again, and sports and physical activities can usually be resumed.

When are the sutures removed?

Sutures are typically removed after 7 to 10 days, depending on the individual healing process.

Will there be visible scars?

Incisions are placed in such a way that scars are usually barely visible—in the natural crease of the upper eyelid, or just below the eyelashes or inside the eyelid for lower eyelid surgery.

How long do the results of eyelid surgery last?

The results of eyelid surgery are long-lasting. While the natural aging process continues, the treated appearance generally remains significantly improved for many years.
